What Testosterone Does in Men: Benefits and Limits
Testosterone is essential for normal sexual development, reproductive function, bone maintenance, muscle, and red blood cell production. More is not automatically better. The health goal is an appropriate physiologic level, not the highest number possible.
Marketing often describes testosterone as a shortcut to strength, energy, confidence, and fat loss. That framing mixes three different situations: normal testosterone physiology, treatment of medically confirmed hypogonadism, and use of supraphysiologic androgens for performance. The benefits and risks are very different.
What testosterone does in the body
Sexual development and function
During puberty, testosterone contributes to development of male reproductive tissues, facial and body hair, voice changes, and increased muscle and bone mass. In adulthood it helps maintain libido and supports normal erectile physiology, although erections also depend heavily on blood vessels, nerves, medications, mental health, and relationships.
Sperm production and fertility
Testosterone produced inside the testes is necessary for sperm production. This does not mean that prescribed or illicit testosterone improves fertility. External testosterone can suppress luteinizing hormone and follicle-stimulating hormone, reducing intratesticular testosterone and sperm production. Men who want current or future fertility need a different clinical conversation.
Muscle and strength
Testosterone supports muscle protein synthesis and maintenance of lean mass. Men with true hypogonadism may gain lean mass when treated, but improvements in strength and physical function vary. In men who already have normal levels, pushing testosterone above the normal range is not a general wellness strategy and can introduce serious risks.
Bone health
Testosterone and its conversion to estradiol help maintain bone. Long-term deficiency can contribute to low bone mineral density. Treatment decisions still require a broader fracture-risk assessment that includes age, smoking, alcohol use, medications, nutrition, exercise, and other hormonal conditions.
Red blood cell production
Testosterone stimulates red blood cell production. Correcting deficiency can improve unexplained anemia in some men, but treatment can also raise hematocrit too much. That is why blood counts are monitored during testosterone therapy.
When treatment may help
Professional guidelines support testosterone therapy for men who have compatible symptoms and unequivocally, consistently low testosterone after appropriate evaluation. Potential benefits may include improved sexual symptoms, bone density, lean mass, and anemia in selected patients. Response is not guaranteed, and treatment should be monitored.

Why higher is not always healthier
High-dose testosterone and anabolic steroids can suppress fertility, shrink the testes, raise hematocrit, worsen acne, alter cholesterol, and contribute to mood and cardiovascular complications. Even prescribed testosterone requires screening and monitoring. Men with certain prostate or breast cancers, elevated hematocrit, untreated severe sleep apnea, recent heart attack or stroke, thrombophilia, or near-term fertility plans may not be appropriate candidates.
The Endocrine Society's 2026 review of current evidence found no meaningful increase in heart attack or stroke in the major TRAVERSE trial over one to four years, but pulmonary embolism and fractures were more common and longer-term safety remains uncertain. That is a reason for individualized care, not reassurance that unsupervised use is safe.
Can lifestyle raise testosterone?
Exercise, adequate sleep, and nutritious food are important for overall health, strength, body composition, and sexual function. In men with obesity-related functional low testosterone, meaningful weight loss can improve levels. Exercise training does not reliably raise resting testosterone in men whose levels are already normal, but it improves health and performance through many other pathways.
Correcting a true nutrient deficiency can restore normal physiology. Taking extra zinc, magnesium, vitamin D, or herbs does not reliably push testosterone higher when nutritional status is already adequate. A 2023 systematic review found that most proposed testosterone-boosting ingredients failed to increase total testosterone.
Questions to ask before trying to raise testosterone
- Do I have specific symptoms or only a desire for better performance?
- Were my levels measured twice in the early morning?
- Could sleep apnea, obesity, medication, illness, or calorie restriction explain the result?
- Do I want children now or later?
- What will be monitored if treatment begins?
Bottom line
Testosterone is vital, but its benefits come from normal physiology and appropriate treatment of confirmed deficiency. It is not a score to maximize. Accurate diagnosis, attention to reversible causes, and monitored care are safer than chasing a higher number with boosters or unsupervised hormones.
