Testosterone and Aging: What Is Normal?
Testosterone often declines as men age, but age alone does not establish testosterone deficiency. Healthy older men can have a wide range of levels, and symptoms commonly blamed on “low T” may also reflect sleep problems, medication, obesity, diabetes, depression, anemia, or normal changes in health and activity.

The distinction matters because labeling every age-related change as a hormone disease can lead to unnecessary treatment while missing other causes. More specific clues include:

How testing changes with age
Testing should still begin with at least two early-morning, fasting total-testosterone measurements on separate days. The assay should be accurate and reliable.
Free testosterone may add information when total testosterone is near the lower limit or when sex hormone-binding globulin is altered. Depending on the pattern, LH and FSH help distinguish a testicular cause from a pituitary or hypothalamic cause.
Testing during acute illness, severe sleep loss, or aggressive calorie restriction can produce a temporarily low result. Correcting reversible factors and repeating the test may prevent misdiagnosis.
Weight and metabolic health
Obesity, particularly central adiposity, is associated with lower total and free testosterone. Longitudinal research suggests that weight gain can intensify decline, while improvements in weight and metabolic health can reverse part of the associated hormonal change in some men.
The goal is not extreme leanness. A sustainable reduction in excess body fat, resistance training, adequate protein, and control of diabetes or metabolic disease can support overall health and normal hormone function.
Sleep and recovery
Sleep apnea becomes more common with age and can contribute to fatigue, sexual problems, cardiovascular risk, and hormone disruption. Loud snoring, witnessed pauses in breathing, morning headaches, or daytime sleepiness deserve evaluation.

Exercise for aging men
